Why Does My Shoulder Hurt? Understanding the Different Causes of Shoulder Pain
Shoulder pain is one of the most common reasons people search online for answers before ever seeing a clinician — and it’s easy to understand why. The shoulder is the most mobile joint in the body, capable of moving through an enormous range of directions, but that mobility comes at the cost of stability. A whole network of tendons, muscles, and bursae work together to keep it moving smoothly, which also means there are a lot of different structures that can become a source of pain.
The trouble is, most shoulder pain feels broadly similar from the inside: an ache around the outside of the upper arm, a catch or pinch when reaching overhead, discomfort lying on that side at night. But the underlying causes can be quite different from one another, and they don’t all respond to the same treatment. Understanding the main categories is a useful first step — but as we’ll get to, it’s not usually enough to self-diagnose and self-treat from there.
Rotator Cuff Related Pain
The rotator cuff is a group of four muscles and their tendons that wrap around the head of the humerus, holding it centred in the shoulder socket and controlling rotation. Rotator cuff related pain is extremely common, particularly from the mid-30s onwards, and can range from mild tendon irritation through to partial or full tears.
It often develops gradually, linked to repetitive overhead activity — think painting, swimming, throwing sports, or manual work above shoulder height — though it can also follow a specific incident like a fall onto an outstretched arm. Pain is typically felt on the outer upper arm, often worse with overhead reaching or lying on the affected side, and there can be an associated feeling of weakness, particularly when lifting the arm out to the side.
Subacromial Impingement
Impingement describes a mechanical pinching of soft tissue — usually the rotator cuff tendons or the bursa that sits above them — between the head of the humerus and the underside of the acromion, a bony ledge at the top of the shoulder blade. It tends to produce a characteristic “painful arc”: discomfort that appears partway through lifting the arm overhead, and eases again once the arm is fully raised or lowered.
Impingement and rotator cuff pathology overlap significantly, and in practice they’re often part of the same underlying picture rather than two entirely separate problems — poor movement control of the shoulder blade, tightness in the back of the shoulder capsule, or weakness in the cuff itself can all narrow the space the tendons have to glide through, leading to repeated irritation.
Biceps Tendon Issues
The long head of the biceps tendon runs up through a groove at the front of the shoulder before attaching inside the joint, and it’s a frequent source of anterior shoulder pain that’s often mistaken for a purely muscular issue. Biceps tendinopathy can occur on its own, but it very commonly appears alongside rotator cuff and impingement problems, since the tendon sits close to those structures and is affected by similar mechanical stresses.
Pain is usually felt at the front of the shoulder, sometimes radiating down towards the elbow, and can be aggravated by lifting, carrying, or resisted elbow bending. Because the biceps tendon rarely acts entirely in isolation, treating it without considering what else might be going on in the shoulder often gives incomplete results.
Why “Internet Exercises” Can Make Things Worse
With so many possible causes producing similar symptoms, it’s tempting to search “shoulder pain exercises” and start working through whatever comes up. This is one of the most common ways we see shoulder problems get worse rather than better.
The issue isn’t that exercise is the wrong approach — targeted, progressive loading is genuinely one of the most effective treatments for most shoulder pain. The issue is that the right exercise depends entirely on what’s actually going on. A generic rotator cuff strengthening routine might be exactly what one person needs, and exactly the wrong thing for someone else whose pain is driven more by stiffness, poor scapular control, or an irritated bursa that needs calming down before it can tolerate loading at all.
Overhead exercises performed too early, or with poor technique, can aggravate an already irritated tendon or bursa. Resistance exercises chosen without regard to the specific movement pattern that’s provoking someone’s pain can reinforce the very compensation strategies that caused the problem in the first place. And because shoulder pain often takes weeks to meaningfully settle either way, it can be a while before someone realises the exercises they’ve been doing diligently every day have actually been holding them back rather than helping.
The Case for a Full Assessment
This is really the core argument for seeing a clinician rather than self-treating from a generic programme: a proper assessment looks at your specific movement patterns, strength, range of motion, and the exact way your pain behaves, and uses that to work out which structures are most likely involved and what’s driving the problem — not just which structure is sore.
In many cases, a thorough clinical assessment is enough to guide an effective treatment plan. But sometimes the picture isn’t clear-cut, symptoms don’t respond as expected, or there’s a suspicion of a more significant tear or structural issue that needs confirming. In those situations, an ultrasound scan can be genuinely valuable. Unlike a clinical exam alone, ultrasound allows a clinician to directly visualise the rotator cuff tendons, the biceps tendon, and the surrounding bursa in real time, including while the shoulder is being moved — giving a definitive diagnosis rather than an educated best guess, and helping rule in or out things like partial tears, full-thickness tears, or significant bursal inflammation.
When an Ultrasound-Guided Injection Might Help
Occasionally, a shoulder is simply too irritable to make progress with exercise alone — every attempt at loading provokes a flare-up, and rehabilitation stalls before it can really begin. In these cases, an ultrasound-guided injection, typically corticosteroid, can play a useful role. Because it’s guided by real-time imaging, the injection can be placed precisely into the affected bursa or around the specific irritated tendon, rather than relying on surface landmarks alone.
The injection itself isn’t a long-term fix — it doesn’t strengthen a weak rotator cuff or correct poor movement patterns. What it does well is settle down significant inflammation and reduce pain enough that rehabilitation exercises, which are the part that actually creates lasting change, can be introduced and progressed properly. Used this way, as a bridge into rehab rather than a replacement for it, it can genuinely shorten the overall road to recovery.

Shoulder pain rarely comes from a single, simple cause, and rotator cuff pathology, impingement, and biceps tendon issues frequently overlap and influence one another. Trying to match your symptoms to a generic internet exercise routine skips the one step that actually determines whether treatment works: finding out specifically what’s going on in your shoulder, and why.
A full assessment — with imaging and guided treatment where appropriate — gives you an accurate diagnosis and a rehabilitation plan built around your shoulder, not a generic template.
